Here we offer a comprehensive strategy for obesity prevention and sustained weight loss, managed directly by physicians and allied health professionals. We aim to enhance health, COVID-19 resistance, productivity, wages, equity, and reduce injuries, early retirement, and premature death.

For participating employees, a blank version of the Periodic Health Assessment (PHA) form is available here.  This is a comprehensive form which will provide valuable information to physicians, and will take around 30-40 min to complete. It must be completed in one instance and will not be saved if discontinued prior to submission.  Please allocate sufficient time for this process.

This allows for direct receipt by our Electronic Medical Records (EMR), which keeps your healthcare information confidential, and cannot be accessed by your employer.
